Bindery Information

This information is for UMC Library Staff wanting to bind personal monographs or periodicals.

This service is a benefit for UMC Library Staff only.

The instructions, order forms and credit card information form below are all Microsoft Word documents; by clicking on the title of each form, the document will automatically open. You must save a copy of the document to your computer if you wish to reuse any information you enter into the form.

 

Please read the Instructions before you begin.

 

If you wish to pay for binding with a credit card, you will need to fill out the Request to Pay by Credit Card form.
