   Work flow is interrupted, immediate equipment problems
2. Need help or have a regular problem:

   Use this form to: add staff to server shares, to report equipment or software problems that aren't interrupting work flow, ask for help using software.
3. Acquire a piece of software or equipment: Download MS Word file
   Forms require supervisor's signature. LTS may work with you to find alternative solutions, depending on the situation.
